Free Initial Consultation

I offer a free 30 minutes initial consultation that would give me the opportunity to make a brief clinical assessment and determine whether I will be a good match for you. During this consult we will also have a chance to agree on the fee and payment schedule based on the information provided. I also offer sliding scale appointments on a  limited basis for individuals who may not have adequate financial means.

Insurance

I do accept California Blue Shield, TriCare, TriWest, Cigna, LifeSynch, Humana, and Mines Associations.  There are also many insurance companies that offer partial compensation for “out-of-network provider.” Please inquire with your insurance carrier to determine your out-of-network benefits. Upon request, I will provide you with a monthly statement that can be submitted for reimbursement.

Payment Methods

I accept cash payment and personal checks. Based on our initial agreement, payments are due at time of service.  

Cancellation Fee

If you are unable to make your appointment, please notify me within 24 hours prior your scheduled appointment. If less notice is given, you will be billed for your appointment.
